New Patient Appointments To schedule your first appointment, we need to collect some basic information in advance so we can verify your eligibility, prepare your chart, and make the most of your visit. When you contact our office to book as a new patient, please be prepared to provide: • Your first and last name • Your date of birth • Your complete dental insurance information, including: – Insurance carrier / plan name – Insurance ID / member number Once we have this information, our team will promptly confirm your appointment. Appointment Cancellation & Missed Appointment Policy We value your time and ours. To allow us to accommodate other patients who need care, we ask that all cancellations or rescheduling requests be made at least 48 business hours (2 full business days) before your scheduled appointment. If you do not cancel or reschedule within this timeframe, or if you miss (“no-show”) your appointment, a $75 fee will be charged for each hour of the reserved appointment time. For example: For example: • 1-hour appointment → $75 fee • 2-hour appointment → $150 fee This policy helps us maintain a full schedule and keep wait times short for everyone.
No Insurance? We have a solution for you! Check out our Members Benefit plan. For adults 13+: The member benefit plan covers 2 sets of x-rays, 2 exams and 2 basic cleanings per year (every six months). The cost is $350 for the year. You will also receive 25% off all diagnosed dental treatment. Children 12 and under: The member benefit plan covers 2 sets of x-rays, 2 exams, 2 basic cleanings and fluoride per year (every 6 months). The cost is $300 per year. You will also receive 25% off all diagnosed dental treatment. Family Plan: If you sign up 2+ people then you save $25 per each additional person and 30% off diagnosed dental treatment. For example, the cost for Parent 1 would be $350, Parent 2 would be $325, child 1 would be $275 and child 2 would be $275. Some exclusions apply, for example IV sedation. No cash discounts. Our office in Orem is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid as per the contract agreement. As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them. If your insurance covers orthodontic treatment, you will receive the benefit of reduced personal costs. Many insurance policies have a lifetime orthodontic benefit that is distinct from regular dental insurance. Insurance policies vary, so we will review your insurance to determine the appropriate course of action. Once treatment has started, we will file your claims.
